HOW TO ORDER

Counsyl Expanded Carrier Screening

How to complete the Counsyl Expanded Carrier Screening test

You are just five steps away from having test results :
1

Test payment

You can pay for your Counsyl screen test(s) in advance. Each person being tested will need to complete your requisition and pay for a test. The cost of each test is $629, and charges will be incurred at the time of online payment. Tests are refundable up until the time you have your sample collected. You are encouraged to check with your private healthcare insurer to see if coverage is provided. Please note, that funding for certain ethnicity-specific genetic screens do exist and vary by province, likely requiring a referral to medical genetics. Examples of ethnicity-specific carrier screens include (but are not limited to) the Ashkenazi Jewish or French Canadian populations. To learn more, please contact your healthcare provider or Ask.Genetics@LifeLabs.com Please note Counsyl can also be paid for at the time of collection.
2

Complete your requisition form

After purchasing the test, you will receive an automated email with your requisition form. Each person having a Counsyl genetic screen performed must complete a test requisition and have it signed by their healthcare provider.
3

Meet with your healthcare provider to sign the requisition form

Each individual being tested must have the completed requisition form signed by their healthcare provider as well as themselves in the consent box. Requisitions not signed by a healthcare provider will not be processed; LifeLabs will not be able to proceed with testing until a signature is obtained.
4

Provide a blood sample

Individuals being tested can visit any LifeLabs Patient Service Centre to have a blood sample taken. For your convenience, you may book an appointment to reduce waiting time. If you are outside Ontario, British Columbia or Saskatchewan, please contact Ask.Genetics@LifeLabs.com to organize your collection.
5

Get your results

Your test results will be sent directly to your healthcare provider for their review and follow up discussion with you. We recommend that you schedule an appointment with your healthcare provider three weeks after your blood sample is collected to review the results. Through Counsyl, you will also have access to a post-test genetic counselling session with a board-certified genetic counsellor to review your results in more detail. Counselling sessions are available in +200 languages can be accessed by calling 1-(888) COUNSYL or visiting the “Schedule Genetic Counselling” section through the “Patient” tab at http://www.counsyl.com/
