Meet with your healthcare provider to complete & sign the requisition form

Each individual being tested must have the completed requisition form signed by their healthcare provider as well as themselves in the consent box. Requisitions not signed by a healthcare provider will not be processed; LifeLabs will not be able to proceed with testing until a signature is obtained.

Provide a blood sample

Individuals being tested can visit any LifeLabs Patient Service Centre to have a blood sample taken. For your convenience, you may book an appointment to reduce waiting time. If you are outside Ontario, British Columbia or Saskatchewan, please contact to organize your collection.

Get your results

Your test results will be sent directly to your healthcare provider for their review and follow-up discussion with you. We recommend that you schedule an appointment with your healthcare provider two weeks after your blood sample is collected to review the results.
